The program features access to Wellness Coaches coupled with comprehensive, up-to-date health and wellness information. Wellness Advocate educates, motivates and guides employees in making healthy lifestyle changes.

Health Risk Assessment (HRA)

Wellness Advocate utilizes an HRA that is online and completely confidential. The questionnaire helps employees identify risks for leading diseases and conditions such as:

• Heart disease and stroke
• Diabetes
• Osteoporosis

Employees receive an easy-to-understand personalized summary of their risks along with suggestions for reducing risks. In addition, the summary includes factors such as emotional health, dental health, stress, and pain scores. Aggregate reports are available for management.

Wellness Coaching

The program provides unlimited access to Wellness Coaches via telephone, email or online chat, combined with comprehensive, up-to-date, reliable health and wellness information, and other online tools to help members reach their health goals.

Each employee receives:
• Access to a password-protected, secure Web page, available 24/7, 365 days
• One-on-one Wellness Coaching set around specific goals geared to employee interest and needs, (e.g., stress reduction, weight loss, nutrition, smoking cessation)
• Regularly scheduled Coaching sessions by telephone, email or online chat
• The full-spectrum menu that augments Wellness Coaching includes comprehensive health information, such as drug interactions, specific diseases, and more
• Interactive tools include calculators for Body Mass Index (BMI), target heart rate and caloric intake, as well as health trackers to chart blood pressure, cholesterol and other health indicators

Personal Health Record

The Personal Health Record (PHR) is a dynamic, online tool designed so members can easily access and track their medical history. The PHR captures the important health information a physician or emergency personnel might need to make informed decisions regarding a member's care. The PHR is available "anytime, anywhere, in any situation." It can be printed, emailed or faxed at any time.

The Personal Health Record Includes:

• Immunization records
• Personal and family medical history, including allergies
• Special forms for chronic conditions
• Medicine Cabinet keeps track of all medications
